I hereby consent to the release of my written testimonial/images of myself and/or to images of my home, business or property to be used at the discretion of Family Trust Federal Credit Union in news releases, publicity, advertising, electronic medium and other information or promotional material.
Family Trust may copyright and use, reuse, publish and/or republish my comment and photo or video likeness in any media for any reasonable purpose, including any printed matter prepared and distributed by or for the credit union.
I understand I have no guarantee of anonymity and that the story/images may include my name in a caption or be included in material for public viewing. I also understand I do not have any right to inspect or approve any publication, advertisement, copy or other printed matter or its use by or for Family Trust.
I also consent to the use of any written statements or testimonials made by myself to be used in the described fashion above. I relinquish any claims of ownership and am not entitled to monies or any other consideration for the use of my statements, testimonials or images.
I release the credit union, its officers, directors, employees and agents from any and all liability, claims, actions, demands and damages relating to any publication, distortion, blurring, alteration, optical illusion, unintended results or use in composite form or otherwise that may occur or be produced.
